Summary of position: As Director of Social Services you would be responsible for providing social services (social work) to residents. This includes evaluating residents, identifying areas of concern and coordinating services with the interdisciplinary team as well as contacting community resources as needed. The Director of Social Services will have a strong managerial background as he/she will oversee case management services for residents and review resident status.

• Provide social services (social work) to residents and evaluate residents’ needs.
• Identify areas of concern and coordinate services with the interdisciplinary team.
• Contact and coordinate with community resources as needed.
• Oversee case management services for residents.
• Review resident status and progress as part of ongoing care planning.

Required Education And Experience

• Bachelor’s Degree in social work (SW) or social welfare from an accredited academic institution
• At least 1-year experience in health care setting
• Excellent written and verbal communication skills
• Computer proficiency with Microsoft products

Preferred Education And Experience

• Current state license as a Social Worker
• Experience in outpatient rehabilitation preferred
• Experience in a fast-paced environment
• Post-acute care or health care experience preferred
