Job Summary:

Long Term Care Nurse Practitioner in Wisconsin This position involves working in a nursing home facility attached to a hospital, providing direct patient care predominantly for elderly residents with diagnoses such as dementia, frailty, and rehabilitation needs. The role requires staffing on nights with a 12-hour shift schedule, participating in charting, assessments, medication management, and clinical procedures.

• Schedule:
  
  Night shifts, 12 hours per shift, with a minimum of 36 hours per week, every other weekend and holiday, on a block schedule with 4 nurses and 7 aides during day/pm shifts, and 2 nurses with 3-4 aides at night
• Float:
  Float rotation may be required
• Additional Information:
  The role involves clinical duties such as charting, assessments, medication administration, and procedures including IVs, catheters, and ostomy care. The unit is staffed with RN's, LPN's, and CNA's, with physician availability after hours. On-site RT and pharmacy services are available 24/7. A unit-specific orientation varies based on skills.
