American Traveler is hiring an experienced Home Health/Hospice RN requiring a WI or compact RN license and at least 1 year of relevant experience.
Details- Home Health and Hospice setting within a Post Acute care environment
- Primarily geriatric population with occasional pediatric patients
- Common diagnoses include heart disease, COPD, CHF, pneumonia, dementia, stroke, cancer, kidney disease, diabetes, liver disease, and complex wounds
- Performs both routine and OASIS visits, seeing approximately 5-6 patients per day (25-30 points weekly)
- Uses Cerner EMR, which is transitioning to Epic
- CPAP and BiPap encountered occasionally, self-managed by patients; no ventilator care required
- Monday through Friday day shift schedule, 8:00am-4:30pm
- On-call requirement 2 times per week, Monday through Thursday
- Weekend work only required during on-call shifts
- No floating between units, but will provide both home health and hospice visits
- Supported by 3 certified nursing assistants and administrator availability after hours
- Active WI or compact RN license required by start date
- Current BLS (AHA) certification required
- Minimum 1 year of relevant nursing experience required
- Prior home health or hospice experience strongly preferred
- Skilled in chronic disease management, patient/family education, NPWT, ostomy care, central line care, IV administration, and complex wound care
- Hospice experience including pain and symptom management and end-of-life care preferred
- Prior travel nursing experience preferred
- Strong critical thinking abilities required
- Conduct home visits to manage chronic diseases, provide wound care, and support end-of-life hospice care with empathy and compassion
- Participate in weekly interdisciplinary team meetings and case conferences, plus monthly all-staff meetings
- Complete department-specific orientation including 1 day of computer-based training and 2-5 days of field training/shadowing
- Dress code requires solid color scrubs without patterns or logos, and closed-toe, non-slip shoes with socks or hosiery
- Must clock in and out at the facility
- Candidates residing within 80 miles of the facility are considered local, which reduces the applicable pay rate
- Former employees must have an end date at least 6 months prior to consideration for this position
