Are you an experienced MDS Nurse looking to make a meaningful impact? Our compassionate and dedicated team is seeking a motivated and detail-oriented MDS Nurse to lead the assessment and care planning process for our residents.

MDS Nurse What You’ll Do:

• Coordinate and complete the MDS assessments and care plans in compliance with state and federal guidelines

• Collaborate with interdisciplinary team members to ensure accurate documentation and individualized care

• Monitor quality indicators and support initiatives for improving resident outcomes

• Ensure timely submission of MDS assessments to maximize appropriate reimbursement

MDS Nurse What We’re Looking For:

• Active RN license in the state of Ohio

• MDS experience in a long-term care or skilled nursing setting (Required)

• Knowledge of RAI process and PDPM guidelines

• Strong organizational and communication skills

• Ability to work independently and as part of a team
