In-Home Geriatric NP Clinician

Matrix Medical Network is seeking Nurse Practitioners to perform in-home comprehensive health assessments, reviewing medical histories, medications, and risks at the member’s residence.

You’ll work with physicians, case managers, and caregivers to close care gaps, order tests as needed, and provide follow-up education. This role requires NP licensure, board certification, and strong communication skills, with flexible scheduling and the potential for FT opportunities.
