Applicants for initial appointment must be able to demonstrate provision of inpatient or outpatient services reflective of the scope of privileges requested for at least 80 patients in the past 24 months or demonstrate successful completion of a hospital-affiliated accredited residency special clinical fellowship or research in the past 24 months. Experience can be demonstrated in one of the following ways: 1

Applicants who have just completed a residency shall provide a copy of their residency log and/or documentation from their program director attesting that the applicant meets the above criteria. 2

Applicants who are not applying directly out of a residency shall provide quality profiles from hospitals or clinics where they currently have privileges showing clinical activity for the last 24 months including number of procedures performed as applicable. Will provider work solo? If not other providers specialties working with them:
Facility is a level 2 trauma center with a full complement of subspecialty support available Licensed Candidate:
Oregon license is preferred but if the ETA for a new license is reasonable and we have immediate candidates to be sent over we could consider. Please include the ETA in all non-Oregon licensed presents. If a candidate is interested in starter any sooner than 90 days such as October then Oregon license must be in hand. Oregon Med Staff and CVO does not allow for fast tracking without a license in hand.
