Maryville is seeking an experienced full-time
Social Services Coordinator

Responsibilities:
  • Developing assessments and written plans of care to identify the needs of each resident.
  • Assessing the resident's psychosocial and emotional needs.
  • Effectively communicating information concerning a resident’s condition with other departments and government agencies.
  • Ensuring that the medically related emotional andsocialneeds of the resident are met and maintained.
  • Participating in the resident discharge planning, providing information to residents and families of programs available.
  • Serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested time frames.
  • Making referrals and documenting outcomes.
  • A strong commitment to maintaining the confidentiality of all resident care information in accordance with HIPAA Guidelines.
  • Developing assessments and written plans of care to identify the needs of each resident.

Qualifications:
  • Bachelor’s degreeis required; focused inSocial

    Work or other Healthcare related fields required.
  • The ability to pass a background check is required of all Maryville Employees
  • Recent experience in aSocial

    Services role in a nursing home/hospital setting or some other

    Social

    Worker role required.
  • Ability to work in a fast-paced work environment.
  • Excellent communication and customer service skills.
  • Strong computer skills required due to this facility working solely in an Electronic Medical Records environment.
  • Bachelor’s degreeis required; focused inSocial

    Work or other Healthcare related fields required.
