The RN Care Coordinator in the Emergency Department is responsible for managing patient transitions, ensuring quality care, and coordinating resources for effective discharge planning. This role is ideal for experienced nurses who excel in acute and emergency care settings.

• Coordinate post-discharge patient care needs for timely and effective discharges.
• Independently complete and document thorough patient assessments.
• Communicate with patients, families, and the healthcare team to develop care plans.
• Facilitate resolution of issues that present barriers to safe transfers.
• Ensure ongoing contact with interdisciplinary staff for safe patient transitions.
• Collaborate with the healthcare team to meet regulatory and compliance standards.
• Perform duties as requested.
