Position: Community Nurse, RN - Memory Care at HEARTHSTONE SENIOR LIVING Beaverton, OR

Overview

Community Nurse, RN - Memory Care role at Hearthstone Senior Living in Beaverton, OR. We are searching for a caring and compassionate Registered Nurse who enjoys working with memory care residents. As the Community Nurse, RN you will be responsible for administrative duties as well as providing hands-on care for our residents. You will be part of a supportive team working together to serve our residents with love, respect and integrity.

Essential Duties and Responsibilities
  • Input new resident data into the system and update current resident’s record.
  • Participate in care conferences and developing resident’s Service Plan.
  • Coordinate care with outside providers and maintain appropriate documentation.
  • Respond to resident/family concerns identified and reported by Caregivers and Med Techs.
  • Document directions and actions appropriately.
  • Perform direct nursing care as needed.
  • Supervise the medication/chart room:
    • Proper medication distribution by Med Techs
    • Review resident orders for accuracy
    • Responsible for Med Room audit – expiration dates, disposing of narcotic and expired medications.
    • Daily charting. (Timely documentation)
    • Oversee EMAR system.
  • Participates in providing continuing education to staff, as assigned.
  • Perform delegation and teaching.
  • Investigate, evaluate, and document incidents or accidents involving residents and ensure follow-up and update service plan accordingly.
  • Work collaboratively with other care staff and management team.
  • Participate in projects, in-service trainings and committees as assigned.
  • Perform pain, diabetic and supportive device assessments in a timely manner.
  • Responsible for significant change in condition.
  • Update ADLs and communicate changes with physicians and care staff, and families as needed.
  • Follow-up on lab reports and doctor orders.
  • Serves as on-call nurse for Community (on a rotating basis).
  • Other duties as assigned.
