American Traveler is seeking a Home Health RN for a West Portland, OR area position requiring OR licensure and OASIS/HCHB proficiency.

• Home health setting serving patients in West Portland, OR and surrounding areas
• Adult home health patient population
• Visits up to 6 patients per day with a 1:1 ratio and 30 productivity points standard
• Day shift schedule, 8-hour shifts (8:00 AM - 4:30 PM)
• No weekend requirements
• No floating between like units required
• No on-call requirement, though on-call rotation may be discussed during interview
• Documentation includes admission assessments, discharge, recertification, resumption of care, routine visits, and start of care visits
• Performs wound care, medication administration/management, IV and subcutaneous injections, phlebotomy, and pain assessment/management

• Active OR RN license required in-hand at time of submission
• BLS certification required
• Minimum of 1 year of skilled home health experience required
• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in Homecare Homebase (HCHB) charting system required
• Must have own reliable transportation and an active driver's license
• Hospice/Home Health checklist required for consideration
• First-time travelers not accepted

• Charting completed via Home Care Home Base, Remote Client, and the Rover mobile app
• Primary series COVID-19 vaccination plus one booster required
• Business casual attire required; no jeans; scrubs permitted but not mandatory
• System groups clients to minimize drive time, though occasional daily drive radius may reach 60 miles (60-90 minutes total)
• Mileage reimbursed at IRS standard rate
• Free parking provided
• Local candidates accepted within a 50-75 mile radius at the same pay rate
• Facility can cancel up to one shift per pay period
• Orientation begins on Mondays and includes billable training modules
• Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Requested time off must be submitted at the time of application
