Job Summary
:
The Social Care Navigator fun c t i ons a s a n in te g r a l p a rt of the BHMH Services Department's Primary Care integration to ass e ss a nd a ddr e ss p a t i e n t 's needs as related to their resilience/self-determination, ability to access outpatient services and so c i a l det e rmin a nts of h e a l t h.

This individual provides skills training and case management s e rvi ce s and links patients to needed community resources through their work on medical and BHMH teams t o c oordi n a te s e rvi ce s a nd c a re that a d dr e ss their medi c a l, ment a l he a l t h a nd/or oth e r p s y c hoso c ial n e e d s .

They also wo r k with t he Pr i m a r y C a re T ea ms t o outr eac h and wo r k with complex h ea l t h a nd/or p s y c hoso c i a l ne e ds of patients a nd those who a re h i g h ut i l iz e rs of hospital a nd Em e r g e n c y D e p a rtm e nt s e rvi ce s.

Essential Duties and Responsibilities:

• F un c t i on a s a n in te g r a l p a rt of the BHMH Team attending huddles, team meetings and coordinating services with team members for complex patients.
