• Administer Medications:
  Give oral, topical, IV, injectable medications
• Wound/Skin Care:
  Perform wound care/prevention, manage pain, apply dressings
• Monitoring:
  Observe physical and mental health status- track changes & analyze data
• Clinical Procedure:
  Provide catheter/ostomy care, gather specimens etc.
• Care Planning:
  Assist in executing individualized care
• Collaboration:
  
  Strategize care with physicians, therapists, social workers & others
• Family & Resident
  
  Education:
  
  Explain health status, changes and expectations for care
• Documentation:
  Keep a detailed record of health status, all care, progress & interactions
• Team Oversight:
  Oversee & delegate CNA’s ensuring quality care/teamwork

• Licensed to work within the State of West Virginia as a LPN Licensed Practical Nurse
• Able to pass background check, drug screen, professional references check & physical evaluation
• Professional & Clinically Skilled with leadership qualities
