Grace at Work is looking for a compassionate and organized Intake Coordinator to join their team in Bedford, IN. In this vital role, you’ll be the first point of contact for clients and families, helping connect them with the care and support services they need. • Manage the client intake process from initial inquiry through enrollment and onboarding
• Conduct intake screenings, assessments, and interviews with prospective clients and families
• Maintain accurate and up-to-date client records, documentation, and intake forms
• Coordinate with internal care teams to ensure smooth client transitions and service placement
• Respond promptly to phone, email, and in-person inquiries with professionalism and empathy
• Track intake data and generate reports to support organizational goals
  
  Ensure compliance with applicable regulations, policies, and confidentiality standards

• Prior experience in intake coordination, case management, social services, or a related field preferred
• Strong interpersonal and communication skills with a compassionate, client-first approach
• Proficiency in data entry, electronic health records (EHR), or case management software
• Ability to multitask, prioritize, and stay organized in a fast-paced environment
• High school diploma or GED required; associate's or bachelor's degree in human services or related field a plus
• Knowledge of HIPAA and client confidentiality requirements
• Reliable, detail-oriented, and committed to serving vulnerable populations
