Roles & Responsibilities   Assess and evaluate patients to develop individualized treatment plans
Deliver skilled physical therapy interventions to improve mobility and function
Document patient progress accurately and communicate updates to the care team
Collaborate with interdisciplinary team members to coordinate comprehensive care
Educate patients and families on exercises, safe mobility, and home programs
Participate in discharge planning and ensure continuity of care
Contribute to quality improvement initiatives and maintain regulatory compliance
Qualifications   Licensed Physical Therapist in New Hampshire or ability to obtain state licensure promptly
Current CPR/BLS certification (or willing to obtain upon hire)
Strong clinical reasoning and hands-on treatment skills in physical therapy
Comfortable working with adult and geriatric populations in a rehab setting
Excellent communication and teamwork skills; patient-centered and professional
Education   Doctor of Physical Therapy (DPT) preferred or Master of Physical Therapy
Knowledge & Skills   Skilled in assessment, manual therapy techniques, therapeutic exercise, and gait training
Knowledge of rehab documentation, outcomes measurement, and care planning
Ability to prioritize caseload and manage time effectively
Familiarity with safety protocols, fall prevention, and mobility equipment
Comfortable using electronic medical records and following facility policies
