Home Care Nurse – Part time job at 360 SHS. Bedford, NH.

At 360 SHS, we have an exciting part‑time opportunity for a compassionate, mission driven Licensed Practical Nurse (LPN) or Registered Nurse (RN). As a nurse at 360 you will be an integral part of ensuring that we are delivering exceptional care to our clients and families, while providing support to our amazing team of caregivers.

• $33-$39 per hour

• Perform visits with clients and caregivers in the home
• Adjust care plans following visits to reflect any changes
• Evaluate client needs and identify opportunities in which we can provide additional support
• Identify opportunities in which we can empower our caregivers through education and coaching
• Document visit details and follow up notes in a timely, accurate, and comprehensive manner
• Work closely with our Clinical Director to be a professional presence in the community
• Collaborate with other team members and our community partners

• 1-year experience as an LPN or RN in a home care setting
• Current state license as an LPN or RN
• Excellent interpersonal skills
• Valid driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally
