Registered Nurse (RN) Case Manager

Lily Care Hospice is a community-based hospice serving patients and families across southern New Hampshire. We are hiring a full-time RN Case Manager to join our founding clinical team.

• Manage a caseload of hospice patients in private homes and facilities, completing assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the on-call rotation

The RN Case Manager role pays $43.00–$50.00 per hour based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is southern New Hampshire, based out of Bedford, NH.

• A current RN license in New Hampshire or a compact state
• One year of RN experience; hospice, home health, or case management experience preferred
• Current CPR certification
• A valid driver's license
