DMC Primary Care, a physician‑owned independent practice located throughout New Hampshire, has been providing comprehensive family care since 1964. We help each patient achieve the best possible health through innovative, patient‑centered care.
We are seeking a full‑time experienced RN or LPN for a triage position in Bedford, NH. This in‑person role is part of a collaborative team that includes social workers, care managers, and another triage nurse. Hours are Monday‑Friday plus rotating Saturdays.
ResponsibilitiesThe successful candidate will ensure professional, compassionate, and accurate communication with each patient caller. The caller’s symptoms will be assessed and triaged according to established DMC nursing processes and approved guidelines to determine the appropriate level of care or self‑care advice. Additionally, the Triage Nurse will support the nursing team by managing patient messages and communicating necessary information to both patients and providers.
As a DMC Triage Nurse, you will assist with transitions of care activities, including contacting patients after hospital discharge, reviewing discharge instructions, performing medication reconciliation, and updating electronic medical records.
Qualifications- Completion of an accredited RN or LPN program (required).
- 1‑3 years of outpatient primary care or triage nursing experience (preferred).
- NH State licensure as a Registered Nurse or Licensed Practical Nurse (required).
- BLS certification (required).
