LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse

Provide licensed practical nursing care in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and adherence to care plans to promote optimal health outcomes.

Responsibilities:

• Administer medications and treatments as prescribed by licensed healthcare providers.
• Observe, record, and report patient conditions and responses to interventions.
• Follow infection control protocols, including wearing photo badges and presenting  all times.
• Maintain accurate documentation of patient care activities and compliance with facility procedures.
• Collaborate with interdisciplinary team members to develop and update care plans.
• Adhere to vaccination and exemption policies, including COVID-19 vaccination requirements.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Ability to follow detailed instructions and abide by facility safety protocols.
• Strong communication and interpersonal skills.
• Demonstrate flexibility and reliability in a contract setting.
• Must comply with vaccination policies or possess a legitimate exemption.
