Dialysis Clinical Coordinator – Home Therapy – U.S. Renal Care, Inc. (Bedford, TX)

ESSENTIAL DUTIES AND RESPONSIBILITIES- The Home Therapy Clinical Coordinator assists with management and operation of designated Home Therapy Program(s).
- Assumes full responsibility for the program(s) in the absence of the Administrator.
- Coordinates the functions of all departments and develops standards and methods of measuring patient care services, including maintaining a chronological record of services required by ESRD Network Coordinating Council and Medicare.
- Works with the Administrator to maintain thorough documentation in patient records of all treatments, activities, and communications with patients, physicians and other healthcare professionals.
- Promotes the quality management program through education, staff and patient involvement in outcomes, and overall responsibility to achieve corporate quality goals.
- Stays current with federal, state and local laws and regulations, including health‑care professional practice act requirements related to home dialysis program and patient care.
- Assists the Administrator with development, implementation and follow‑up of corrective action plans required for internal and external surveys.
- May assume the Home Therapy Nurse’s responsibilities as needed.
- Conducts and participates in formal and informal meetings with the governing body, Regional Director, Medical Director and delegated staff.
- Ensures staff meetings, in‑service education and patient‑care planning meetings are held monthly as delegated.
- Maintains the Quality Assessment & Performance Improvement Program.
- Oversees patient (and/or care partner) training in Peritoneal Dialysis or Home Hemodialysis practice.
- Ensures proper infection control monitoring, implementation and recording.
- Assists in on‑call system oversight and ensures patients have nursing support at all times.
- Assists with disinfection of equipment and supplies, including bicarbonate delivery systems and dialysis machines.
- Responds to all emergencies in the program and is familiar with emergency equipment and operational procedures.
- Communicates and reviews emergency preparedness procedures with home staff, patients and interdisciplinary team members.
- Provides communication of overall results of patient assessment, reassessment and ongoing monitoring to physicians and team members.
- Ensures staff meet required qualifications for positions and perform duties within the limits of company policy, health‑care practice acts and applicable laws.
- Assists with maintaining effective personnel management and employee relations, including evaluating performance and counseling employees.
- Promotes corporate management goals through team leadership and accountability.
- Licensed Registered Nurse (RN).
- Experience in dialysis care:
- Maryland: at least six months of dialysis experience.
- Texas: at least twelve months of clinical experience and six months of dialysis experience after completing the facility’s training program.
- Dialysis experience must be within the last twenty‑four months.
- A current certification from a nationally recognized board in nephrology nursing or dialysis may substitute for the six months of dialysis experience.
