The Pulmonary Clinic Advanced Practice Provider (APP) is a certified and licensed Nurse Practitioner (NP) or Physician Assistant (PA) who delivers specialized care to chronically ill patients in a clinic setting. Working both independently and in collaboration with supervising physicians, the APP provides comprehensive, patient-centered care, utilizing advanced clinical knowledge and critical thinking skills to diagnose, treat, and manage complex medical conditions.
This role emphasizes evidence-based practice, effective communication, and a commitment to improving patient outcomes.
Clinical Expertise and Patient Care
- Demonstrate a thorough understanding of the etiologies, risk factors, pathophysiology, and clinical presentation of chronic and acute medical conditions.
- Perform accurate and complete problem-focused histories and physical assessments for adult patients.
- Order, collect, and interpret laboratory and diagnostic tests to evaluate patient health status and inform treatment decisions.
- Prescribe and manage pharmacologic agents and other treatment modalities, ensuring efficacy, safety, and adherence to evidence-based guidelines.
- Identify age-specific patient needs and adapt care plans to address these requirements.
- Develop and implement comprehensive, outcomes-based treatment plans using patient assessments, evidence-based medicine, and professional practice standards. Collaborate with the interdisciplinary team when appropriate.
- Monitor patient responsiveness to treatments and adjust care plans as needed to optimize outcomes.
- Maintain open and effective communication with patients, families, and healthcare team members to ensure coordinated care.
- Counsel and educate patients and their families on medical conditions, treatment options, and self-management strategies.
- Document all patient interactions, assessments, and treatments accurately, completely, and promptly in the electronic health record (EHR).
- Conduct oneself in a professional and ethical manner, adhering to established clinical and behavioral standards.
- Participate in internal and external peer review processes to address any concerns related to medical decision-making, competence, clinical judgment, or professional behavior.
- Work collaboratively as a member of a multidisciplinary team, fostering a culture of respect and teamwork.
- Demonstrate the ability to work independently while maintaining appropriate physician collaboration for complex cases.
Education and Certifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Current certification and licensure as an NP or PA.
- Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications.
- Must have a minimum of 2 years of pulmonary experience.
- Preferred 2 years of experience working in a clinic setting as an NP or PA.
- Excellent verbal and written communication skills, with the ability to convey complex medical information clearly and effectively.
