A health services company is seeking an LTSS Service Coordinator-Clinician in Indiana. The role requires an LSW, LCSW, or LMSW and at least 2 years of experience working with individuals with chronic illnesses. The coordinator will assist an RN in case management, conducting assessments, and coordinating care for members. This is a field-based role with a schedule of Monday-Friday, 8am-5pm EST, and flexibility is expected to travel to client sites.
