Job specification

• Lead and manage home health clinical staff, including nurses and therapists, ensuring effective care coordination and team collaboration.
• Supervise day-to-day operations of the home health program, including admissions, scheduling, documentation, and patient care delivery.
• Ensure that all care is compliant with Medicare, state, and agency regulations and policies.
• Monitor patient outcomes and implement quality improvement strategies when needed.
• Collaborate with physicians, discharge planners, and families to facilitate individualized patient care plans.

Candidate requirements

• A bachelor's degree is required; a master's degree is preferred.
• Must hold an active license as an RN, PT, or OT, along with current CPR certification.
• A minimum of 3 years of clinical nursing or patient care experience is required.
• Supervisory or management experience of at least 2 years is preferred.
• Valid driver's license and auto insurance are required due to the nature of home health visits.
