Job Description

Oncology Nurse Navigator, full time, exempt, days, 8:00am-4:00pm

JOB SUMMARY

Under the direction of the Director of Oncology Integrate Health , functions as a clinician, case manager, and educator to achieve optimal clinical and quality outcomes for the oncology population. The Oncology Nurse Navigator (ONN) will coordinate patient care from diagnosis - through treatment and as needed throughout the continuum of care. The ONN will assist transition of services including but not limited to chemotherapy, radiation therapy, surgical care, survivorship and palliative care.

The ONN demonstrates and models a full range of nursing professional practice in both care provided and participation in development of strategies to improve patient outcomes, satisfaction with services and cost effectiveness. The ONN functions as a part of the multidisciplinary team. Serves as a liaison throughout the healthcare system and community.

Qualifications

Education & Training: Registered Nurse with current Maryland License with experience in oncology. Knowledge of nursing theory, practice and education in order to conduct patient triage, intake and assessment and participate in the care management process. Knowledge of adult learning theory, instructional/educational design principles, instructional media, evaluation methods and generally accepted training and development practices.

Work Orientation &

Experience:

Three to five years’ experience in oncology with the advanced skill and knowledge in case management, and the ability to implement sound educational programs. Oncology Certified Nurse (OCN) highly preferred.

Oncology Care Coordination & Navigation
  • Initial and Ongoing Assessment: Complete comprehensive assessments of oncologic patients, serving as the essential link between patients, families, and physicians.

  • Coordinate Treatment & Appointments: Work across disciplines to ensure timely scheduling of appointments, results reporting, and communication regarding patient care and outcomes.

  • Multidisciplinary Clinic Participation: Coordinate and actively participate in multidisciplinary clinics and oncology conferences keeping the entire care team informed of the plan of care and patient progress.

  • Monitor and Consult: Monitor patient progress, consulting with physicians, mid-level providers, and the care team. Refer patients to appropriate services for complex care issues.

  • Continuous Improvement: Initiate, review, and update assessment tools, education materials, care management guidelines, and pathways from diagnosis to survivorship.

Patient/Family Education & Support
  • Disease

    Education:

    Educate and reinforce education with patients and families on disease, procedures, testing results, and next steps based on the multidisciplinary plan of care.

  • Resource Navigation: Assist patients in overcoming barriers to care (e.g., transportation, financial, childcare) by providing referrals to Social Work, Financial Counseling, and other support resources with appropriate follow-up.

  • Material Provision: Provide all necessary education and support material at any Kaufman Cancer Center visit and ensure engagement with established protocols.

Collaborative & Professional Relationships
  • Promote Positive Relationships: Foster positive working relationships with patients, families, and physicians, considering varied needs and values.

  • Team

    Collaboration:

    Maintain collaborative professional working relationships with clinical and administrative team members to ensure a successful cross-continuum of care for the patient population.

Quality Improvement & Outreach
  • Process Development: Assist the Center in developing, implementing, and evaluating new care processes, including improvements in technology, equipment, and best practice guidelines.

  • Outcomes & Performance Improvement: Participate in Center performance improvement activities, tracking patient outcomes and relevant data for measurement.

  • Community Outreach: Assist with conducting and coordinating health education classes for cancer prevention and early detection in conjunction with Community Outreach.
