Job SummaryUnder the direction of the Director of Oncology, the Oncology Nurse Navigator functions as a clinician, case manager, and educator to achieve optimal clinical and quality outcomes for the oncology population. The ONN coordinates patient care from diagnosis through treatment and as needed throughout the continuum of care, assisting transitions of services including chemotherapy, radiation, surgical care, survivorship, and palliative care.
Key Responsibilities- Initial and Ongoing Assessment:
Complete comprehensive assessments of oncologic patients. - Coordinate Treatment & Appointments:
Ensure timely scheduling of appointments, results reporting, and communication. - Multidisciplinary Clinic Participation:
Actively participate in multidisciplinary clinics and oncology conferences. - Monitor and Consult:
Monitor patient progress and consult with physicians and the care team. - Continuous Improvement:
Initiate and update assessment tools, education materials, care management guidelines, and pathways from diagnosis to survivorship. - Patient/Family Education & Support:
Provide disease education, resource navigation, and material provision during visits. - Collaborative & Professional Relationships:
Foster positive relationships with patients, families, and physicians; maintain collaborative professional relationships with clinical and administrative team members. - Quality Improvement & Outreach:
Assist in developing, implementing, and evaluating new care processes; participate in performance improvement activities; assist with community outreach health education classes.
- Registered Nurse with current Maryland license.
- Three to five years’ experience in oncology with advanced case management skills.
- Oncology Certified Nurse (OCN) highly preferred.
- Knowledge of nursing theory, practice, and education; adult learning theory and instructional design principles.
- Pay Range: $38.67 - $58.05 per hour.
