Position Overview

We are seeking an experienced Registered Nurse Case Manager for an inpatient care management assignment. This role focuses on coordinating safe patient discharges, managing complex social and medical cases, overseeing guardianship needs, and facilitating appropriate post-acute placements and community resources.

The ideal candidate will have strong acute care or inpatient case management experience, excellent care coordination skills, and the ability to collaborate with interdisciplinary teams to ensure optimal patient outcomes.

    8:00 AM - 4:30 PM (may vary based on department needs)
  • 40 Hours per Week

    Approximately 15 patients daily

    None
  • Weekend Requirement:
    Not required, but additional shifts may be available
  • Holiday Requirement:
    Required as scheduled
  • Floating Required Between Nearby Locations

Key Responsibilities
  • Complete initial and ongoing patient assessments.
  • Develop and coordinate individualized plans of care.
  • Facilitate discharge planning and transitions of care.
  • Coordinate medical home care, placements, guardian ships, and community resources.
  • Collaborate with physicians, social workers, nursing staff, and interdisciplinary teams.
  • Advocate for patients and ensure access to appropriate services.
  • Identify psychosocial needs and connect patients with available resources.
  • Perform chart reviews and documentation audits as needed.
  • Participate in case conferences, quality assurance activities, and utilization review activities.
  • Maintain accurate and timely patient records.
  • Support safe discharge planning through coordination of medical and social services.
  • Serve as a liaison between patients, families, providers, and community agencies.
Patient Population

This inpatient case management role supports patients requiring:

  • Complex discharge planning
  • Skilled nursing and rehabilitation placement
  • Long-term care placement

  • Community resource coordination
  • Medical home care services
Requirements Required
  • Active Maryland RN License
  • Minimum 3 years of Case Management experience
  • Inpatient hospital or acute care experience
  • Epic charting experience
  • Strong discharge planning and care coordination skills
  • Excellent communication and interdisciplinary collaboration skills
Preferred
  • BLS Certification
  • Previous travel nursing experience
  • Experience with guardianship and placement coordination
