Description

Position Requirements
  • Degree:
    Bachelor’s or Master’s in Psychology, Sociology, Social Work, Nursing, Counseling, Human Services Administration, or related field
  • Valid driver’s license and insured vehicle
SCI Summary

SCI supports people with disabilities, behavioral challenges, medically complex needs, transition youth, the elderly, and other populations using Maryland Department of Health’s targeted case‑management and supports‑planning work models.

Position Summary

Provide case‑management services and support to individuals requiring assistance with activities of daily living (e.g., bathing, dressing, toileting, eating, mobility). Core responsibilities include establishing positive relationships, exploring options, developing and monitoring person‑centered plans, meeting state‑mandated requirements, and ensuring provider compliance with standards.

Essential Duties
  • Advocate for clients, provide education and resources, and promote self‑determination
  • Collect eligibility information and documentation, work with clients and representatives, maintain updated plans
  • Develop and write person‑centered plans, conduct joint preparation sessions, and continually update tools
  • Research, refer, and coordinate services to meet plan outcomes; assist with transitions to community from institutional settings
  • Monitor plans, conduct visits, track progress, complete required reports, and re‑apply for programs as necessary
  • Adhere to SCI policies: meet performance benchmarks, maintain HIPAA records, use tracking tools

Minimum Qualifications

  • Experience with person‑centered planning (preferred)
Physical Demands

Standard office environment; must remain stationary >95% of time, use computer, keyboard, mouse, and mobile device. May occasionally lift 10–25 lbs. Requires vision for close work, ability to operate a motor vehicle, and effective communication.
