  • Core responsibilities:

    Deliver high-quality, patient-centered care to long-term residents, including routine assessments, medication administration, and timely documentation. Monitor vital signs, identify subtle changes in condition, and escalate concerns per protocol. Collaborate with physicians, RNs, and the broader care team to implement individualized care plans, ensuring safety, fall prevention, wound care, and supports for activities of daily living. Maintain accurate records, update care plans, participate in discharge planning where appropriate, and support families through education and compassionate communication.
