LVN ​/ LPN – LTC​/SNF

Long-Term Care Nurse

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure patient safety, administer medications, and monitor vital signs following established protocols.

Responsibilities:

• Assess patients’ health conditions and document findings accurately.
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and respond to any changes in patient status.
• Collaborate with healthcare team to develop and update care plans.
• Educate residents and their families on health management and procedures.
• Maintain compliance with facility policies, safety standards, and infection control protocols.

Qualifications:

• Hold a valid LVN or LPN license in the state of practice.
• Demonstrate reliable clinical skills and understanding of long-term care procedures.
• Possess current CPR and Basic Life Support (BLS) certification.
• Exhibit strong communication and teamwork abilities.
• Comply with agency requirements for COVID-19 vaccination or approved exemptions.
• Bring valid state or federal  be prepared to wear a photo badge at all times.
