LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) Or Licensed Practical Nurse (LPN)

Seeking a dedicated Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) for a local contract position providing quality patient care in an LTC/SNF setting. The role involves collaborating with healthcare teams to ensure optimal patient outcomes.

Responsibilities:

• Administer patient assessments and monitor vital signs to evaluate health status.
• Provide nursing care and treatments following physician orders and facility protocols.
• Document patient records accurately and in a timely manner.
• Communicate effectively with patients, families, and healthcare team members.
• Ensure compliance with infection control and safety standards.
• Maintain professional certification and adhere to facility policies.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to comply with vaccination requirements or approved exemptions.
• Strong interpersonal and communication skills.
• Ability to work flexible schedules, including a 5-week contract.
• Must bring government-issued  wear a photo badge on shift.
