Long-Term Care/Skilled Nursing Facility Nurse

Provide skilled nursing care to residents in long-term care or skilled nursing facilities, ensuring their health, safety, and comfort. Administer medications and treatments, monitor patient conditions, and collaborate with healthcare teams to deliver comprehensive care.

Responsibilities:

• Assess residents’ health status and document observations.
• Administer prescribed medications and treatments accurately.
• Monitor vital signs and respond to changes in patient condition.
• Collaborate with nurses, physicians, and other healthcare providers.
• Maintain detailed and timely medical records.
• Ensure compliance with facility protocols and safety standards.

Qualifications:

• Valid LVN or LPN license in the state.
• Minimum of 1 year of experience in LTC or SNF settings.
• Ability to accurately administer medications and perform clinical tasks.
• Strong communication and interpersonal skills.
• Knowledge of infection control protocols and safety procedures.
