LVN​/LPN LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in a skilled nursing or long-term care setting. Ensure patient safety, comfort, and compliance with all healthcare standards and protocols.

Responsibilities:

• Administer medications and treatments accurately following physician orders and facility policies.
• Monitor residents’ health status and report changes or concerns to the healthcare team.
• Document patient care activities thoroughly and in a timely manner.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Ensure compliance with safety protocols, infection control standards, and legal regulations.
• Assist residents with activities of daily living, including hygiene, mobility, and nutrition.

Qualifications:

• Valid LVN or LPN license in the state of New Mexico.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to perform clinical tasks accurately and efficiently.
• Strong communication skills and the ability to work collaboratively.
• Knowledge of healthcare laws, regulations, and safety standards.
• Current CPR/BLS certification.
