Registered Nurse II – Case Manager – Outpatient, Medical Group Practices – Nursing. Req #: 80826. Flexible full‑time 32 hours per week M‑F.

The MH Registered Nurse II – Case Manager – Outpatient role manages a caseload of patients at high risk of adverse outcomes. Responsibilities include coordinating care, removing barriers, connecting patients with resources, data mining, patient education, group sessions, health and wellness promotion, and care transitions.

Sign‑on bonus up to $10,000 for RN with 0‑2 years of experience and $20,000 for RN with >2 years of experience. Bonus prorated for part‑time hires; per diem hires ineligible.

• Graduate of an accredited School of Nursing (Bachelor’s degree preferred).
• Valid state RN license, valid driver’s license, and BLS certification (or obtain within 30 days).
• Minimum 2 years of acute care RN experience.
• At least 1 year of recent utilization review, medical necessity, and clinical record review experience.
