Netwerkggzregionw Vl zoekt een Trajectbegeleider Mobiel Crisisteam Oostende-Veurne. Je voert outreach en urgentiezorg uit, triageert telefonisch en werkt nauw samen met politie, huisarts en spoeddiensten. Je missie is crisissituaties stabiliseren en ziekenhuisopname voorkomen.
Je hebt een bachelor in zorgberoep of VVAZ en minimaal 3 jaar GGZ-ervaring. Rijbewijs B en flexibiliteit zijn vereist; er staan dienstwagens/e-bikes en een laptop ter beschikking.
