Summit Home Care and Hospice is searching for an experienced and motivated Registered Nurse (RN) to join our Home Health team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

• Hours:
  
  Monday - Friday;
  Standard Business Hours & Occasional On-Call Rotation
• Annual Salary: $75,000 - $85,000 per Year (Overtime Eligible)
• Location/Territory:
  Bellefontaine, OH & Surrounding Areas
• Work Setting:
  On the Road for Patient Visits & Training at the Local Office or Remotely
• Expected Travel: ~30-Mile Radius (Mileage Reimbursed)

• The Registered Nurse (RN) plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience.
• The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

• Completes an initial assessment of patient and family to determine home care needs.
• Provides a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change. Patient Care
