- The Staff Registered Nurse is responsible for assessing patient/family/caregiver needs, planning care, implementing the Plan of Care, and evaluating effectiveness.
- Pay: $39.21 hourly, full‑time, 40 hours weekly / 80 hours bi‑weekly.
- Schedule:
Monday–Friday 8:30 AM–5:00 PM, with a rotational on‑call schedule that includes evening, weekends, and holidays. On‑call is limited. - Location:
Based at the Belleville Patient Services Office, serving St. Clair, Monroe, Randolph, and Washington counties. - Valid driver’s license, auto insurance, and reliable transportation are required; mileage reimbursement is $0.70 per mile.

- Graduate of a state‑approved school of nursing.
- Associate Degree in Nursing or Diploma in Nursing required.
- One year experience in hospice preferred.
- Current Illinois nursing license.
- Certification in hospice and palliative care preferred.
- CPR certification preferred.
- Leadership, managerial, clinical assessment, organizational, and interpersonal skills.
- Working knowledge of Medicare/Medicaid, hospice regulations, and third‑party payor expectations.
- Reliable transportation.
- Maintain a caseload of patients.
- Provide direct patient care, including on‑call after‑hours support.
- Perform assessments, medication reviews, symptom and pain evaluation; enter medication orders and obtain physicians’ orders.
- Communicate findings to the interdisciplinary team via phone, encrypted text, and IDT meetings.
- Review and update the Plan of Care each visit.
- Supervise Hospice Aide plan of care, ensuring visits every 14 days per Medicare regulation.
- Provide patient and family education on disease progression and symptom management.
- Identify and address physical, psychosocial, emotional, and spiritual needs.
- Document assessment details timely, following policies.
- Oversee Licensed Practical Nurse activities.
- Participate in on‑call triage, emergency decision‑making, and crisis response.
- Offer emotional, spiritual, and psychosocial support, establishing therapeutic relationships.
- Share updates at IDT meetings at least every 15 days.
- Collaborate with team, embrace change, manage stress, maintain positive attitude.
- Contribute to referrals, QAPI activities, and performance improvement.
- Be a self‑starter, flexible, motivated, and meet deadlines; complete required education on time.
- Incorporate core values; manage paid time off and complete timesheet documentation.
- Other duties as assigned.
- Perform repetitive tasks such as bathing, transferring, lifting (up to 100 lbs.) and turning.
- Work 20 % office time in an air‑conditioned environment and 80 % traveling to patient homes.
- Potential exposure to odors, infectious fluids, heat, and traffic.
- Requires vision better than 20/20 over 20 feet, hearing alarms, ability to handle stress and work in distractions.
