Position Summary
:
Under the supervision of a Registered Nurse, and in collaboration with the Interdisciplinary Team (IDT), provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy.
Pay
: $28.72 hourly. Full‑time, non‑exempt position.
Schedule
: 40 hours weekly, Monday‑Friday 8:30 AM‑5:00 PM. Includes a limited rotational on‑call schedule for evenings, weekends, and holidays, with on‑call time limited.

Based out of the Belleville, IL Patient Services office. Primarily serves patients in Belleville’s office northern territory, which includes the following Illinois counties:
Clinton, Madison, Bond, Montgomery, Macoupin, Jersey, and Calhoun.
Visit locations include patients’ homes, long‑term care facilities, nursing homes, hospitals, etc. Valid driver’s license, current automobile insurance, and reliable transportation are required. Company reimburses $0.67 per mile for personal vehicle use.

RequirementsEducation
- Graduate of a state‑approved school of nursing.
- Successful completion of Licensed Practice Nurse education.
Experience
- One (1) year experience in hospice preferred.
Other Qualifications
- CPR certification preferred.
- Excellent clinical assessment, organizational, and interpersonal relationship skills.
- Working knowledge of Medicare/Medicaid, hospice regulations, and third‑party payor expectations preferred.
- Reliable means of transportation.
Physical Requirements
- Performs repetitive tasks, including bathing, transferring, lifting, and turning patients. Exerts up to 100 lb occasionally and up to 50 lb frequently.
- Requires physical flexibility for bending at the knee and squat‑bend. Must be able to sit for prolonged periods.
- Vision >20/20, hears alarms, telephone, and tape recorder.
Working Conditions
- Spends approx. 20% of time in an air‑conditioned environment and 80% in an automobile.
- Potential exposure to malodorous, infectious body fluids, allergens, cleaning agents, communicable diseases, and traffic.
- Collaborates in developing and modifying the comprehensive nursing plan of care with the Field Staff Nurse/Case Manager.
- Performs physical assessment, collects data, and collaborates with the Field Staff Nurse/Case Manager in assessing the patient’s health status.
- Implements aspects of the plan of care as delegated.
- Participates in health teaching and counseling to promote optimal health.
- Advocates for the patient/family/caregiver, communicating with the IDT as delegated.
- Evaluates patient responses to interventions and provides direct patient care.
- Participates in on‑call after‑hour care as needed; triages calls and provides care under RN direction.
- Demonstrates basic LPN clinical skills; performs annual skills day.
- Applies knowledge of disease progression at end‑of‑life and provides education.
- Practices within Illinois Department of Professional Regulation guidelines.
- Provides evidence‑based physical, emotional, psychosocial, and spiritual care under RN direction.
- Knows principles of end‑of‑life pain and symptom management.
- Monitors symptoms and reports changes to the Field Staff Nurse/Case Manager.
- Works efficiently with minimal supervision; is self‑starter with flexibility, motivation, initiative, and commitment.
- Participates in IDT meetings, providing information about patient’s health and plan of care.
- Collaborates with others to meet deadlines, goals, and objectives; assists case managers with visit frequency and hospice quality measures.
