Hospice Field Staff Licensed Practical Nurse (LPN)
Location:
Belleville, IL office. The LPN will primarily visit patients and travel within the Belleville offices service area, covering the following counties:
Clinton, Madison, Bond, Montgomery, Macoupin, Jersey, and Calhoun.
- Under the supervision of a Registered Nurse and in collaboration with the Interdisciplinary Team, provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy.
- Pay: $28.72 hourly. Full‑time, non‑exempt position.
- Schedule:
Monday‑Friday, 8:30 AM – 5:00 PM, with a rotational on–call schedule that includes some evenings, weekends, and holidays.
- LPN credential.
- Valid driver’s license, current automobile insurance, and reliable transportation. Company reimburses 0.70 cents per mile for driving in a personal vehicle.
- Experience providing hospice or palliative care is preferred.
- Collaborate positively in the development and modification of the Field Staff Nurse/Case Manager comprehensive nursing plan of care for hospice patients.
- Perform physical assessments, collect data, and collaborate with nursing staff to assess health status.
- Implement aspects of the care plan as delegated.
- Participate in health teaching and counseling to promote optimum health of patients.
- Advocate for patient/family/caregiver by communicating and collaborating with the IDT.
- Evaluate patient responses to interventions.
- Provide direct patient care.
- Participate in on‑call after‑hours care as needed; triage calls and provide care under RN direction.
- Demonstrate basic LPN clinical skills and attend required annual skills day.
- Show knowledge of end‑of‑life care, pain and symptom management, and provide evidence‑based, emotional, psychosocial, and spiritual care under RN direction.
