Hospice Field Staff Licensed Practical Nurse (LPN)

Hospice of Southern Illinois is a not-for-profit, free-standing, community-based hospice offering services for people at the end of life across Southern Illinois and St. Louis Metro-East. This position is based out of our Belleville, IL office and primarily serves patients in the northern service territory (Clinton, Madison, Bond, Montgomery, Macoupin, Jersey, and Calhoun).

Position Overview
  • Position Summary: Under the supervision of a Registered Nurse, and in collaboration with the Interdisciplinary Team, provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy.
  • Pay: $28.72 hourly. Full-Time Non-Exempt Position.
  • Schedule: Monday–Friday 8:30 AM–5:00 PM with rotational on‑call.
  • Location: Belleville Patient Services Office. Visits patients in homes, long-term care facilities, nursing homes, hospitals, etc.
  • Valid driver’s license, current automobile insurance, and reliable transportation required; mileage reimbursed at $0.70 per mile.

Position Requirements
  • A.

    Education:

    Graduate of a state‑approved nursing school. Successful completion of Licensed Practice Nurse education.
  • B.

    Experience:

    One year experience in hospice preferred.
  • C. Other

    Qualifications:

    Current Illinois LPN license. CPR certification preferred. Excellent clinical assessment, organizational and interpersonal skills. Knowledge of Medicare/Medicaid, hospice regulations, and third‑party payor expectations preferred. Reliable transportation.
  • Physical Requirements: Performs tasks such as bathing, transferring, lifting, and turning of patients. Exerts up to 100 lb occasionally; up to 50 lb frequently; up to 20 lb constantly. Requires flexibility at knee and squat‑bend. Works in air‑conditioned rooms and in automobiles. May encounter malodorous body fluids, allergens, cleaning agents, and communicable diseases.
  • Working Conditions: Approximately 20 % time in air‑conditioned environment, 80 % driving. Exposure to noise, humidity, and potential hazards.
Essential Functions
  • Collaborates in developing and modifying the comprehensive nursing plan of care.
  • Conducts physical assessment, collects data, and collaborates with the Field Staff Nurse/Case Manager.
  • Implements delegated aspects of the plan of care.
  • Provides health teaching and counseling.
  • Advocates for patients and families, communicating with the IDT.
  • Monitors patient responses and reports changes.
  • Participates in on‑call care, triages calls under RN direction.
  • Demonstrates basic LPN clinical skills and performs annual skills day.
  • Educates patients/families about end‑of‑life pain and symptom management.
  • Maintains documentation and reports to the Field Staff Nurse/Case Manager.
  • Works efficiently with minimal supervision; self‑starter.
  • Participates in IDT meetings, quality measures, and QAPI activities.
  • Adheres to Illinois Department of Professional Regulation guidelines.
  • Manages paid time off and time‑sheet documentation.
  • Other duties as assigned.
