Hospice of Southern Illinois, a not‑for‑profit hospice, is hiring a Hospice Field Staff RN/Case Manager to work at the Belleville office and visit patients throughout the Southern Illinois and St. Louis Metro‑East region.

Position Summary
  • Staff RN evaluates patients, families, and caregivers; plans and implements care plans; participates in the inter‑disciplinary team.
  • Manages patient needs and care coordination from admission to discharge.
Compensation & Work Schedule
  • Pay
    : $39.21 per hour, full‑time non‑exempt, 40 hours per week.
  • Schedule
    :
    Monday‑Friday 8:30 AM‑5:00 PM with rotational on‑call evenings, weekends, and holidays.
  • Location
    :
    Belleville Patient Services Office; travel to patient homes, long‑term care, nursing homes, hospitals, etc., requiring a valid driver’s license, insurance and reliable transportation (reimbursement $0.70 per mile).
Key Responsibilities
  • Maintain a caseload of patients and coordinate care with Hospice Aides and the IDT.
  • Provide direct patient care, including medication orders, pain and symptom assessment, and physical examinations.
  • Communicate findings to the hospice physician and obtain medication orders.
  • Update and review the Plan of Care on each visit and supervise Hospice Aide visits every 14 days.
  • Educate patients, families, and caregivers on end‑of‑life care and pain management.
  • Work on‑call when the regular nurse is unavailable.
Qualifications & Requirements
  • Registered Nurse with current Illinois RN license.
  • Experience in hospice or palliative care is preferred.
  • Strong clinical skills and demonstrated ability to work independently.
  • Excellent communication skills and ability to work collaboratively in a multidisciplinary team.
