• The Staff Registered Nurse is responsible for assessing the patient, family, and caregiver needs, planning care, implementing the Plan of Care, and evaluating its effectiveness.
• As a member of the interdisciplinary team, the RN/Case Manager oversees the direction and coordination of the patient’s care throughout their time in hospice.
• Pay
  : $39.21 hourly. Full‑time non‑exempt, 40 hours weekly.

  Monday‑Friday 8:30 AM‑5:00 PM with an additional rotational on‑call schedule that includes evenings, weekends, and holidays.

  Based at the Belleville Patient Services Office, primarily visiting patients within the Belleville service area (St. Clair, Monroe, Randolph, Washington counties).
• Valid driver’s license, current automobile insurance, and reliable transportation are required. Mileage reimbursement of $0.70 per mile for personal vehicle use.

• Graduate of a state‑approved school of nursing.
• Associate Degree or Diploma in Nursing required.

• One (1) year of hospice experience preferred.

• Current Illinois nursing license.
• Certification in hospice and palliative care preferred.
• CPR certification preferred.
• Excellent leadership, managerial, clinical assessment, organizational and interpersonal skills.
• Working knowledge of Medicare/Medicaid, hospice regulations, and third‑party payor expectations.
• Reliable means of transportation.

Perform repetitive tasks—bathing, transferring, lifting, and turning patients; sit for prolonged periods; exert up to 100 lbs occasionally, 50 lbs frequently, and 20 lbs constantly. Requires flexibility to bend at the knee and squat‑bend. Must manage stress, make decisions under pressure, and handle multiple priorities. Requires clear vision within 20 inches to 20 feet, and hearing alarms, phone, and tape recorder.

Approximately 20% of the time is spent in an air‑conditioned environment with variable humidity and noise; 80% in a patient’s home or automobile. Exposure to malodorous odors, infectious bodies, pet hair, allergens, noxious cleaning agents, communicable diseases, and traffic may occur.

• Maintain a caseload of patients and oversee the direction and coordination of their care and that of family and caregivers.
• Provide direct patient care, including all aspects of the nursing process, and participate in on‑call after‑hours care.
• Demonstrate required registered nurse clinical skills and exhibit them at the annual skills day.
• Complete a physical assessment, medication review, and symptom/pain evaluation on each visit; enter medication orders in a timely manner; communicate findings to the hospice physician and obtain orders; order medications from pharmacy promptly.
• Communicate pertinent information to all disciplines via communicator, telephone, encrypted text messaging system, and at the IDT meeting.
• Review and update the Plan of Care on each visit.
• Initiate, supervise, and update the Hospice Aide (HA) plan of care in a timely manner; supervise visits at least every 14 days per Medicare regulation.
