Position Summary
Under the supervision of a Registered Nurse and in collaboration with the Interdisciplinary Team (IDT), the Hospice Field Staff LPN provides direct and indirect patient care in accordance with the Practice Act and organizational policy.
Pay & SchedulePay: $28.72 hourly. Full‑Time, Non‑Exempt.
Schedule:
Monday–Friday 8:30 AM–5:00 PM with a rotational on‑call schedule that includes evenings, weekends, and holidays. On‑call is limited.
Based out of the Belleville Patient Services Office. Visits patients in homes, long‑term care facilities, nursing homes, hospitals, and other care settings. Must have a valid driver’s license, current automobile insurance, and reliable transportation. Reimbursement: $0.70 per mile for personal vehicle use.
Responsibilities- Collaborate in the development and modification of the comprehensive nursing plan of care for the hospice patient with the Field Staff Nurse/Case Manager.
- Perform physical assessments, collect data, and collaborate on the assessment of patient health status.
- Implement aspects of the nursing plan of care as delegated.
- Provide health teaching and counseling to promote optimal health of patients, as delegated.
- Advocate for the patient, family, and caregiver by communicating and collaborating with the IDT.
- Participate in the evaluation of patient responses to interventions.
- Deliver direct patient care and participate in on‑call after‑hour care as needed, including triaging calls and providing care under the direction of a Registered Nurse.
- Demonstrate basic LPN clinical skills and provide a practical exhibition at the annual skills day.
- Apply knowledge of end‑of‑life disease progression, pain and symptom management, and provide evidence‑based physical, emotional, psychosocial, and spiritual care under the direction of a Registered Nurse.
- Monitor symptoms and changes in patient conditions, documenting and reporting as appropriate.
