Position Summary:
The staff registered nurse is responsible for assessing the patient/family/caregiver needs, planning for the care, implementing the plan of care, and evaluating the effectiveness of the care provided. As a member of the IDT, the staff registered nurse/Case Manager oversees the direction and coordination of a patient's care throughout their time in hospice.
Pay: $39.21 hourly. Full-time Non-Exempt Position. 40 hours weekly / 80 hours bi-weekly.
Schedule:
Monday - Friday 8:30 AM - 5:00 PM with an additional rotational on-call schedule that includes some evening, weekends, and holidays. On-call would be limited.
Location:
Belleville Patient Services Office. Primarily visit patients and travel within the Belleville service area, including St. Clair, Monroe, Randolph, and Washington counties.

Travel:
Valid driver's license, current automobile insurance, and reliable transportation are requirements of the position. The company reimburses 70 cents per mile for driving personal vehicles.

Essential Functions of the Field Staff RN/Case Manager Position:
- Maintains a caseload of patients.
- Oversees the direction and coordination of the patient's care and the care provided for their caregiver/family.
- Provides direct patient care that includes all aspects of the nursing process and participation of on-call after hours care.
- Work on-call in absence of on-call nurse and when need arises.
- Demonstrates required registered nurse clinical skills.
- Gives a practical exhibition and explanation at the required annual skills day.
- On each visit completes a physical assessment, medication review, and a symptom and pain evaluation.
- Accurately and timely enters medication orders.
- Communicates findings to the hospice physician and obtains orders to manage care.
- Timely ordering of medications from pharmacy.
- Communicates pertinent information to all disciplines by communicator, telephone, encrypted text messaging system, and at the IDT meeting.
- Reviews and updates the plan of care (POC) on each visit.
- Responsible for initiating, supervising and updating the hospice aide (HA) plan of care (POC) on a timely basis and supervising visits at least every 14 days per Medicare regulation.
- Knowledgeable in patterns of disease progression at end of life and provides effective ongoing patient/family/caregiver education.
