Job Purpose &

Position Overview:

This position serves as one of the clinic RNs for the Wa I pi nagu Program. The Wa I pi nagu Program is a part of the Winnebago Public Health Department (WPHD), a division of the Winnebago Comprehensive Healthcare System (WCHS). This position is responsible for working closely with the clinic team following the ADA Standards of Care in Diabetes and utilizing the Chronic Care Model.

This position requires becoming a Certified Diabetes Care and Education Specialist (CDCES) to ensure skill level in diabetes prevention, and care management. The position includes keeping up with training and qualifications to align with the goals of the public health department to deliver culturally grounded care for the community.

Specific Job Duties/Responsibilities:

  • Provide direct diabetes nursing duties with the program provider.
  • Conduct assigned duties by the Program Director when providing diabetes-nursing outreach to the service population.
  • Participates as a member of the multidisciplinary diabetes and wellness team to assess community health needs and provide culturally relevant care and education for diabetes. Coordinates all teaching and patients care efforts with members of the team.
  • Provides nursing care and education primarily for at-risk patients and their families and for patients living with diabetes or at risk for diabetes and family members in the community, home, and field clinics.
  • Provides direct patient education, including guidance and outreach to individuals, and clinic or class participants regarding treatment regimens and necessary lifestyle adaptations designed to manage diabetes, prevent complications, and/or prevent its onset.
  • Records and monitors clinical data and assesses individual education needs. Coordinates with the clinical staff. Writes, updates, and revises diabetes care plans with patients in collaboration with the Wa I pi nagu clinical team.
  • Serves as a role model, teacher, and nursing consultant to the Whirling Thunder Wellness Program and other Winnebago Comprehensive Healthcare System teams on all aspects of diabetes care including clinical care of patient and professional diabetes education.
  • Participates in routine record maintenance in compliance with the Privacy Act.
  • Coordinates the interdisciplinary team approach for delivering specialty diabetes teaching to the patient with pregnancy and diabetes, the patients diagnosed fewer than five years, and patient groups targeted for diabetes prevention following the Chronic Care Model.
  • Participates and coordinates events and activities in the Winnebago community as needed for the Wa I pi nagu Program.

Qualifications/

Skills:

  • Knowledge of professional nursing principles and procedures with clinical practice, experience, and training.
  • Knowledge of Diabetes Standards of care in the management of Diabetes.
  • Knowledge of the chronic care model
  • Knowledge of diabetes education concepts.
  • Ability to communicate effectively both verbally and in writing with a scope of people including other health professionals, agencies, and patients.
  • Knowledge of availability and use of community resources and referral processes.
  • Work is performed within the established plan, policies, and objectives of the Wa I pi nagu (Diabetes) Program and the standards of care for patients with Diabetes.
  • Must have a valid driver's license and must be able to meet driver's eligibility guidelines.
  • Active Registered Nurse License.
  • Must be a certified Diabetes Care and Education Specialist or eligible to take the certification within 1 year.
  • Minimum of one (1) year in public health or related field.
