Physical Therapist (PT) – Facility SettingNew Graduates Welcome!
Position SummaryWe are seeking a compassionate and motivated Physical Therapist (PT) to join our interdisciplinary care team in a facility-based setting
. This role is ideal for new graduates or experienced therapists who are passionate about helping patients regain mobility, independence, and confidence through personalized rehabilitation plans.
You’ll work closely with residents, families, and clinical staff to deliver high-quality therapy services that improve functional outcomes and overall quality of life.
Key ResponsibilitiesEvaluate patients and develop individualized treatment plans based on physician orders and patient goals
Provide physical therapy services focused on strength, balance, mobility, gait training, and functional independence
Educate patients and caregivers on exercises, safety techniques, and home programs
Document evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory standards
Collaborate with nursing, therapy, and interdisciplinary teams to ensure coordinated care
Monitor patient progress and adjust treatment plans as needed
Maintain compliance with state practice acts, facility policies, and ethical standards
QualificationsGraduate of an accredited Physical Therapy program
Licensed or license-eligible Physical Therapist in the state of practice
New graduates encouraged to apply
