Hillcrest Health & Living is looking for an experienced, detail-oriented, and clinically strong MDS Coordinator (RN) to join our clinical team at Hillcrest Health & Rehab in Bellevue, NE.

Hillcrest Health & Rehab is Bellevue’s only skilled nursing facility, providing short-term rehabilitation, long-term care, and post-acute services in a supportive, resident-centered environment. Our clinical team works together to support quality outcomes, regulatory compliance, accurate documentation, and strong resident and guest care.

As the MDS Coordinator, you will play an important role in supporting that mission by coordinating the MDS assessment process, ensuring accurate documentation, supporting care planning, and helping drive quality and reimbursement outcomes.

What You’ll Do

As the MDS Coordinator (RN), you will coordinate and complete the MDS assessment process in accordance with state and federal regulations. You will work closely with nursing, therapy, leadership, and interdisciplinary team members to support accurate documentation, timely submissions, and resident-centered care planning.

You will:

Coordinate and complete MDS assessments in accordance with state and federal regulations.

Ensure MDS documentation and submissions are accurate, complete, and timely.

Collaborate with nursing, therapy, social services, dietary, activities, and leadership teams to support resident care planning.

Monitor documentation to support quality care, regulatory compliance, and appropriate reimbursement.

Review Medicare and Medicaid requirements and support compliance with reimbursement guidelines.

Monitor case mix index and identify opportunities for improvement.

Support regulatory readiness and participate in audits, surveys, and quality review processes.

Provide education and guidance to clinical teams on MDS processes, documentation standards, and reimbursement requirements.

Partner with clinical leaders to identify documentation gaps and support timely follow-up.

Maintain strong knowledge of MDS, RAI process, Medicare/Medicaid regulations, and skilled nursing documentation expectations.

Communicate clearly and professionally with interdisciplinary team members, residents, families, and leadership.

Support a culture of accuracy, accountability, compliance, and resident-centered care.

What We’re Looking For

We are looking for an RN who is organized, accountable, and confident with MDS coordination in a skilled nursing environment.

The right candidate will be detail-oriented, self-directed, and comfortable working across multiple disciplines to ensure documentation is accurate, timelines are met, and care plans reflect resident needs. This role is a strong fit for someone who understands the importance of compliance, reimbursement, teamwork, and follow-through.

Ideal candidates will have:

Active RN license in Nebraska or a compact state required.

Previous MDS experience required.

RAC-CT certification preferred.

Strong knowledge of Medicare, Medicaid, reimbursement, MDS, and regulatory requirements.

Skilled nursing facility or long-term care experience strongly preferred.

Strong attention to detail with documentation, assessments, care plans, and submission timelines.

Excellent communication skills with nursing, therapy, leadership, and interdisciplinary team members.

Ability to identify documentation gaps and follow up with the appropriate team members.

Strong organization skills with the ability to manage deadlines and competing priorities.

A self-directed work style with strong accountability and follow-through.

A collaborative, team-first approach focused on quality care and compliance.

Comfort using technology, clinical systems, and documentation resources efficiently and effectively.

Education & Credentials

Active Registered Nurse license in Nebraska or a compact state required.
