The Social Service Assistant functions under the guidance and supervision of the Social Service Director to provide social services that assist residents in maintaining or improving their ability to manage physical, mental, and psychosocial needs. This role supports residents and families through care coordination, advocacy, documentation, and emotional support in a skilled nursing environment.
Reporting:
This position reports directly to the Social Service Director.
** Key Responsibilities
** Assist with the admission process including facility tours, inquiries, and interviewing residents and families Coordinate personal care needs such as hair services, transportation, adaptive equipment, clothing, and personal items Maintain communication with families and physicians regarding care plan meetings Document all social service contacts and referrals accurately Assist in resolving grievances, theft, and loss issues Support residents/families with forms and applications (advance directives, Medi-Cal, voter registration, etc.)
Participate in behavior management and quality assurance programs Attend in-service trainings and staff meetings Promote and protect resident rights in all interactions Ensure compliance with state, federal, and facility regulations## What We’re Looking For
** Qualifications & Skills
** High School Diploma or equivalent required Classes or experience in social work or related field preferred Knowledge of long-term care regulations and social service standards Strong written and verbal communication skills Ability to communicate effectively with residents, families, and staff Understanding of confidentiality and HIPAA compliance Exceptional customer service and empathy Ability to prioritize tasks and meet deadlines Ability to handle emotional stress in a healthcare setting Basic math skills (addition, subtraction, multiplication, division) Knowledge of emergency and disaster procedures
** Physical & Working Conditions
** Skilled Nursing Facility environment Primarily indoor work with normal temperatures Frequent sitting, walking, and movement throughout facility Must be able to lift and carry up to 25 pounds Exposure to occasional odors, chemicals, and blood borne pathogens May experience emotional stress associated with long-term care, including emergency response, resident behaviors, family concerns, and end-of-life situations Weekend and holiday shifts may be required
