Home & Community Program

Overview

Rehab Without Walls® is seeking an experienced Occupational Therapist to join our Home & Community Program in Seattle. This is a W2 PRN role, offering flexibility with the stability, support, and collaboration of an interdisciplinary care team. Our model brings outpatient-level occupational therapy directly to clients in the environments where daily life actually happens.

What You’ll Do
  • Deliver functional occupational therapy in home and community settings
  • Support clients as they transition back to work, driving, and increased community participation
  • Address real‑life skills such as daily routines, community mobility, and independence with meaningful activities
  • Collaborate with an interdisciplinary team including PTs, SLPs, and LCSWs

Responsibilities
  • Perform evaluations and develop treatment plans
  • Regularly re‑assess effectiveness of treatment plans, attend staffing meetings, and family conferences
  • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
  • Communicate patient’s needs and progress to the treatment team, physician, person receiving services, and family members
  • Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes
Qualifications
  • Minimum of a bachelor’s degree in Occupational Therapy from an accredited program
  • Current, unrestricted license as an Occupational Therapist in the state of practice
  • Current CPR Certification
  • A minimum of one year’s work experience as an Occupational Therapist
  • Preferred knowledge of rehabilitation techniques related to complex neurological diagnoses
  • Effective professional communication in verbal and written interactions
  • Ability to lift 50 pounds; lifting patients up to 100 pounds following safety procedures
  • Requires fine motor skills, visual acuity, and ability to walk/stand for extended periods
  • Physical requirements:

    pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently
  • Health screen or examination may be required prior to assignment and periodically thereafter, per local and state regulations
Benefits and Compensation

USD $50.00 - $60.00 / Hour
