RN Case Manager

Professional Case Management is seeking a compassionate and detail-oriented RN Case Manager to join our Healthcare & Medical Services team. In this role, you will provide in-home, patient-centered care to individuals with complex medical needs, primarily former energy workers and their families. You will be responsible for coordinating and delivering clinically appropriate nursing services in the home, completing comprehensive assessments, developing and updating individualized care plans, and monitoring outcomes to ensure the highest quality of care.

As an RN Case Manager, you will collaborate closely with physicians, therapists, social workers, caregivers, and internal care coordination teams to ensure seamless, integrated services for each client. You will oversee and coordinate clinical interventions, manage medications and treatments, educate patients and families on disease management and equipment use, and advocate for appropriate resources and benefits. Your strong clinical judgment, critical thinking, and communication skills will support safe transitions of care, proactive identification of changing conditions, and timely updates to the care plan.

This position offers the opportunity to build meaningful, long-term relationships with clients in their homes while exercising autonomy in managing your daily schedule and caseload. You will document all assessments, visits, and communications accurately and promptly in accordance with organizational, payer, and regulatory requirements, maintaining compliance with all healthcare regulations and privacy standards. You will participate in case conferences, quality improvement initiatives, and ongoing training to continually elevate the standard of care.
