Position Summary
Providence at Home with Compassus, leaders in home‑based hospice and palliative services, is seeking a Hospice Registered Nurse (RN). The RN will provide person‑centered care in the home environment, assess patient and family needs, deliver professional nursing care grounded in the full nursing process, and collaborate with our Interdisciplinary Group (IDG) to develop and direct individualized plans of care. The role focuses on delivering dignity and comfort during end‑of‑life care, organizing and directing hospice care, and incorporating community health education.
Shift: Evening/Night Shift – 3:30 pm to 8:00 am. This is a per‑diem position.
Position Specific Responsibilities- Complete assessment of patient and family to determine hospice needs.
- Provide a complete physical assessment and history and deliver professional nursing care using all elements of the nursing process.
- Write and initiate Plan of Care in collaboration with the IDG.
- Regularly reevaluate patient/family/caregiver needs and revise the Plan of Care as necessary.
- Develop a care plan with patient/family/caregiver and IDG, establishing goals based on nursing diagnosis and incorporating palliative nursing actions.
- May administer medications and treatments as prescribed by the IDG‑approved Plan of Care.
- Participate in on‑call duties as defined by on‑call procedures.
- Maintain productivity standards as set by organization guidelines.
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient condition and care given.
- Supervise ancillary personnel and delegate responsibilities when required.
- Participate in orientation, education, and development of team members.
- Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through ongoing education and in‑service classes.
- Required:
Associate or Bachelor’s degree from an accredited School of Nursing. - Preferred: 1 year experience as a registered nurse. Experience as a hospice CNA or hospice LPN will also be considered.
- Required:
Current, unencumbered (State of Employment) License as a Registered Nurse. - Required – Within 30 days of hire – National Provider BLS – American Heart Association (Vendor Managed).
Effective performance of essential functions requires regular standing, walking, and manual dexterity. The RN may need to lift or move objects up to 50 pounds and has standard visual acuity requirements. Exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Adaptations can be made for individuals with disabilities.
CompensationRange
$40.04–$72.09 per hour. Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
