Job Title

Overview

The Clinical Program Coordinator will complete hospital and nursing orientation at CNMC, coordinate nursing care and services for patients and families, and consult with physicians and other healthcare providers to resolve health concerns. The role requires adherence to legal scope of practice, national and specialty nursing standards, CNMC Policies and Procedures, and applicable laws and regulations. The coordinator leads the patient care team for a specific patient population, assessing, planning, implementing and evaluating clinical care to ensure continuity of care.

Key Responsibilities
  • Coordinate nursing care and services for patients and families in colorectal surgery through telephone, electronic, in-person, or mail.
  • Consult with physicians and other healthcare providers to resolve health concerns.
  • Lead the activities of the patient care delivery team for the target population.
  • Provide continuous accountability to assess, plan, implement and evaluate clinical care.
  • Participate in shared nursing leadership and support the sustainability of nursing excellence.
  • Contribute to internal/external committees, professional organizations, and community activities.
  • Participate in the division of nursing’s performance improvement processes.
  • Identify and report on nurse‑sensitive indicators (NSIs) for ambulatory nursing practice.
  • Integrate evidence‑based practice and research into care planning.
  • Assist physicians and other providers in maintaining appropriate cost and desired patient outcomes.
  • Speak up about unsafe behaviors, continuously validate information, and act promptly to resolve uncertainties.
  • Communicate accurately, clearly and timely, both verbally and in writing.
  • Promote safety for patients, families, visitors, and co‑workers.
  • Plan and collaborate with patients, families and interdisciplinary staff for shared decision‑making.
  • Develop and implement nursing interventions based on patient responses and evidence‑based guidelines.
  • Provide health education and cultural competency support throughout the patient journey.
  • Coordinate mechanisms across systems to provide continuity of care and advise on policy and protocol development.
  • Foster interdisciplinary patient care rounds and conferences to optimize outcomes.
  • Ensure accurate scheduling and coverage, and participate in cross‑coverage when needed.
  • Direct patient and staff flow to maintain a smooth care environment.
  • Collaborate with RNs, LPNs, PCTs and other staff within state practice guidelines.
  • Escalate clinical or administrative issues to proper resolution levels and participate in peer review.
  • Contribute to a positive work environment, demonstrate flexibility, and identify improvement opportunities.
Qualifications
  • Minimum Education:

    Bachelor's Degree (Required)
  • Minimum

    Work Experience:

    5 years of nursing experience, with at least one year of recent pediatric experience, and at least one year of demonstrated nursing leadership (charge nurse, educator, preceptor, council chair/member, or comparable). (Required)

Schedule & Compensation

Part‑time position: 20 hours per week, 10‑hour shifts.

Work hours:

7:00 a.m.–5:30 p.m.

Full‑time salary range: $82,347.20 – $ per year.
